Requirements
The Seller shall include a Certificate of Conformance (CofC) with each shipment for articles identified on the shipping document. If the Seller is not the manufacturer or service provider, a manufacturer’s or service provider’s CofC shall also be included with the Seller’s CofC.
The following shall be included on the Seller’s and/or Manufacturer's CofC and/or shipping document with each shipment:
- Seller's name and address.
- Buyer’s purchase order number and line item.
- The part number and dash number, as specified by the purchase order.
- Quantity of items shipped as specified on the purchase order.
- Country of Origin (optional if this information can be obtained through other means).
- Serial numbers, date codes, or lot/batch numbers (as applicable).
- Original Equipment Manufacturers (OEM) name and address (when different from the Seller).
- Signature or stamp with the title of the Seller’s authorized personnel signing the certificate, date of signing, and a statement attesting that articles provided under this contract conform to all contract requirements. Certificates utilizing secured electronically-generated signatures with the title of the Seller’s authorized personnel are acceptable.
- Revision level/version to which the articles were manufactured or serviced.
- Serial numbers, lot numbers, batch codes, or date (as applicable).
- When multiple lot/batch numbers are delivered in a single shipment, the Seller shall separate and identify the lot/batch numbers and indicate each lot quantity.
